Authorization & Waiver of Dispute Rights
By completing and signing this form, you (the "Cardholder") make the following legally binding acknowledgments to Prestige Hyperbaric (Prestige Hyperbaric and Wellness LLC) (the "Company"):
- You authorize the Company to charge the amount stated below PLUS a 4% credit card processing fee to the credit card you identify. The 4% fee is charged in addition to (not included in) the approved purchase amount, and applicable taxes may apply.
- You understand that hyperbaric chambers and related equipment are custom-manufactured to order, and that once production begins the order is non-cancellable and non-refundable.
- You waive your right to dispute, reverse, or charge back this transaction, except for those fraud protections that the card networks are required by law to provide.
- You agree that any bad-faith dispute or chargeback filed on a legitimate, authorized, delivered, or in-production order will make you liable for all resulting fees, collection costs, and reasonable attorneys' fees.
- You agree that your electronic signature below is a legally binding signature under the Florida Uniform Electronic Transactions Act and has the same effect as a handwritten signature.
- You agree that this authorization is governed by the laws of the State of Florida, with exclusive jurisdiction and venue in Pasco County, Florida.
Do not sign until you have read the Credit Card Terms and Payment Terms. Once production begins, your order cannot be cancelled or refunded.
